2.         Job Purpose

            Provides comprehensive healthcare sensitive to the needs of the

            population, including assessment, diagnosing, prescribing, and

            referrals where appropriate. Enabling patient choice in provision of care,

            complementing roles of GP and multidisciplinary team and promoting

           development of advanced nursing practice.

           

 

 

3.         Dimensions

          

           The prime function of the post is-

            Providing holistic health care for patients including those with

            undifferentiated and undiagnosed problems using advanced

            nursing skills. Practice list size 3,800 increasing to 5,000

8 P/T GPs, 2 P/T NPs, 2 P/T PNs, 1 P/T HCA, & 7 admin staff.

 

Directing and participating in provision of continuing care, health

maintenance, screening and health promotion to patients across

whole practice population as part of patient centred multidisciplinary team.

 

Conducting physical and psychological assessments to diagnose patients’ conditions and provide appropriate self care advice, prescribing treatment, or referral using evidence based guidelines.

 

Managing and participating in Chronic disease management, family planning and telephone triage as part of a multidisciplinary team.

 

Communication with PHC team and PCT and attendance at meetings, supporting and developing work towards agreed PMS targets.

 

Utilize educational knowledge and skills within practice and for educational placements/ continual professional development for health professionals.

Lead nurse for managing Mental Health patients working as part of a multidisciplinary team.

5.         Knowledge, Skills And Experience Required

 

            First level registered Nurse

            Post registration qualification- Nurse Practitioner First or Master’s degree

            Level. Minimum of three years Primary Health Care post registration

            experience. Teaching qualification (minimum equivalent 998 and

            experience.), and accredited training in Chronic disease management,

            women’s health and extended Nurse prescribing qualification desirable.

            Ability to manage own caseload as part of a team. Excellent

            Communication and interpersonal skills. A willingness to demonstrate

            Flexibility and innovative ways of working to achieve patient centred

            care. Good organisational and leadership skills. Demonstrable clinical

            competence. Evidence of personal/ professional development.

 

 

 

 

 

 

6.                 Key Result Areas (Not Prioritised)

 

1.        Contribute to the effective management of holistic patient centred care by

           use of diagnostic, triage, and assessment skills, health promotion,

          screening, chronic disease management, and nursing procedures.

 

2.       Provide clinical assessment, diagnosis, and treatment interventions using

          well developed evidence based clinical decision making and judgement

          skills, and demonstrating benefits of Extended Nurse Prescribing.

            

3.      Ensure patient data is accurately recorded and audited in line with Clinical

          Governance to achieve audit and research requirements for PMS.

 

4.      Contribute to development and maintenance of local health needs

         assessment in collaboration with the wider Healthy Living Centre.

 

5.      Contribute to the provision of multidisciplinary education, student

         mentoring and promotion and development of nursing roles.

 

6.       Participate in meetings to implement and enable effective integrated team

          working in practice and community team, using leadership skills to support

          nurse leadership within practice.

 

7.       Participate in development of Practice Clinical guidelines, and using skills

         and experience to promote knowledge for patients and staff.

 

8.      Maintain own clinical competencies and professional development

         commensurate with job requirements.
